Unusual presentation of sarcoidosis in a patient with latent TB

This is a report of a patient with latent tuberculosis (TB) who presented with isolated cutaneous, psoriaform sarcoidosis with scarring alopecia of the scalp. The patient had a unique presentation with salmon-colored, hyperkeratotic, and eroded plaques on his arms, back, stomach, and face. The scalp initially presented with extensive ulceration with a secondary bacterial infection that evolved into scarring alopecia. Multiple punch biopsies identified non-caseating granulomas with negative AFB staining, confirming the diagnosis of sarcoidosis. The patient received hydrocortisone 2.5 % topical cream to apply to his plaque-like lesions, and doxycycline twice a day (BID) and mupirocin 2% ointment for his scalp wound. In addition, he continued treatment of his latent TB with rifampin. The patient will be evaluated by his ophthalmologist and started on hydroxychloroquine therapy.
